LPN-Patient Navigator - Diabetes Management

We Are Hiring:
Ascension Providence is hiring a LPN/LVN
This is a full-time role working 8A-4:30P,
What You Will Do:
- LVN required
- 3 years of professional medical experience required.
- Prefer experience interacting with other medical
Serve as a patient navigator to accommodate
patients throughout the continuum of care.
What You Will Need:
- Coordinate and lead presentations to prospective patients which
may include community seminars, expositions and support
- Provide support with navigating the healthcare system and
assists patients with utilizing available community support
- Address barriers to care. Work with Navigation Team as needed
and serve on hospital committees as assigned.
- Work with physicians and the health care team to accommodate
patient issues and to ensure quality patient care.
Licensure / Certification /
- BLS Provider credentialed from the American Heart Association
(AHA) obtained within 3 Months (90 days) of hire date or job
transfer date required.
- Licensed Clinical Practical/Vocational Nurse credentialed from
the Texas Board of Nursing required.
- High school diploma or GED required.
No additional preferences.
