This role focuses on delivering physical therapy services to pediatric patients in their homes, working to improve mobility, manage pain, and achieve developmental goals. Our ideal candidate is compassionate, detail-oriented, and driven by a commitment to excellence and patient success in a home-based setting. Evaluate patient condition and formulate individualized patient care plans. Implement physical therapy interventions as prescribed by a physician. Regular assessment of patient progress and modification of therapy plans as needed. Educate family members on patient care and therapy routines Maintain patient records and compliance with healthcare regulations. Collaborate with multidisciplinary team members to ensure comprehensive care. Expert knowledge of pediatric physical therapy techniques and equipment Strong interpersonal and communication skills Ability to work autonomously in a home care environment Proficient with patient management software and EMR systems Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current state licensure as a Physical Therapist (PT) Valid Driver’s License and reliable transportation CPR certification
