Job Description:
----- - Registered Nurse Case Manager role providing direct
patient care and coordinating home health services.
Primarily serves Onondaga County, with occasional visits to
surrounding areas.
Conducts home visits Monday-Friday, 8a-4:30p, with one
weekend/month; no on-call required.
Develops care plans based on assessments and OASIS evaluations,
ensuring documentation meets quality standards.
Supervises LPNs and home health aides, guiding patient care and
team collaboration.
Maintains compliance with policies, quality initiatives, infection
control, and regulatory standards.
Requires NYS RN license, valid driver--------s license, and
computer proficiency; previous home care experience is a plus.
Physical demands include patient transfers, lifting, and driving in
various weather conditions.
