**Titers must be in hand at time of submission Patient Ratio: 2 to
1 AVERAGE SCRUB COLOR - Royal Blue TRAVELERS SUBMITTED WITHOUT THE
NECESSARY INFORMATION OR REQUIREMENTS WILL BE REJECTED MUST INCLUDE
-PERMANENT HOME ADDRESS -AND -SS# -ON THE SUBMISSION TRAVELERS MUST
BE -GREATER THAN 50 MILES -FROM FACILITY
