We can't wait to welcome you to the Strive Team!_*$2000 Bonus Eligible Position for Full-Time*_*JOB DUTIES** Assess home health pediatric patient for rehabilitative and functional status.* Regularly re-evaluate the patients/clients occupational therapy need and evaluate the outcomes of care on a regular basis.* Occupational Therapist develops, initiates, and revises the plan of care as necessary to ensure quality and continuity of care.* Identifies deficits including but not limited to strength, fine motor coordination, sensory dysfunction, motor planning and self-care, including feeding and the potential for intervention. Evaluates the home environment.* Occupational Therapist evaluates the need for and recommends DME devices and aides.* Uses infection control measures that protect both the staff and the patient (OSHA).* Coordinates services.
